• Brokers must be appointed with PacifiCare to sell new business.
• Your name, agency name, address, phone and fax numbers
• Group name, address, phone and fax numbers
• Type of plan to be quoted (For example, full-service or eyewear-only vision)
• Complete roster of employees, including all dependent information
• Employer contribution and claims history
